Does the smaller head have a mouth useable for oral, like the default head? I also noticed that I see no option for fleece yarn hair. The Fleece hair looks too much like Medusa for my tastes. Thank you.

The smaller head has the same mouth as the regular head, ready to serve ;-)
Fleece yarn hair is the same price as other fleece hairs, you just specify the type and color after you place the order. She'll still turn part of you to stone though :D :D
